Prioritization Frameworks: ABCs, Maslow and Nursing Process
Many nursing students feel paralyzed when faced with multiple "correct" answers on the NCLEX, often because they lack a systematic way to rank patient needs. Using NCLEX prioritization frameworks provides a clear, logical structure to evaluate patient stability and ensure safety remains the primary focus.
The Hierarchy of Needs: ABCs and Beyond
Airway, Breathing, and Circulation (ABCs)
The ABCs serve as your first line of defense. Always prioritize life-threatening conditions. If a patient has an obstructed airway, that takes precedence over an IV medication or even cardiac monitoring.
Maslow’s Hierarchy of Needs
Once physiological needs are stabilized, move up the pyramid:
- Physiological: Oxygen, circulation, nutrition (The ABCs live here).
- Safety: Fall precautions, infection control.
- Love/Belonging: Support systems.
- Esteem/Self-Actualization: Patient education and coping.
Acute vs. Chronic
Always prioritize the acute change. A patient with a sudden onset of dyspnea is a higher priority than a patient with chronic stable COPD who has a baseline slight wheeze.
The Nursing Process: A Decision Tool
The Nursing Process (ADPIE) isn't just for care plans; it is a clinical judgment framework. When a question asks what to do first or next, assess the clinical situation:
- Assessment: Do you have enough information? If you haven't assessed, you cannot act.
- Diagnosis/Analysis: Identify the problem.
- Planning/Implementation: Take action.
- Evaluation: Check the results.
Pro-tip: If a patient is exhibiting new or unusual symptoms, your first step should almost always be to Assess (e.g., auscultate lung sounds) before jumping to Implement (e.g., calling the provider or administering meds).
Actual vs. Potential Problems
A common mistake is prioritizing a potential risk over an actual, active problem.
The Rule of Urgency
- Actual Problem: A patient currently vomiting is a higher priority than a patient at risk for aspiration.
- Safety First: If a patient is at risk for injury (potential) but another patient is currently experiencing respiratory distress (actual), the respiratory patient wins every time.
Always differentiate between what is happening right now versus what might happen later in the shift.
Applying The Blueprint Method to Prioritization
Using our proprietary Blueprint Method, you can dismantle complex questions systematically:
- Read: Identify the stem. Is it asking for the 'first' action or 'most important'?
- Summarize: What is the underlying clinical condition?
- Classify: Is this an ABC issue, a safety issue, or an assessment need?
- Evaluate: Compare the options against the framework.
- Decide: Choose the option that stabilizes the patient or prevents harm.

Book Your NCLEX TutorFrequently asked questions
Should I always use ABCs over Maslow?
Yes, in acute care settings. The ABCs represent the physiological survival layer of Maslow's hierarchy, which is always the top priority.
When is it appropriate to assess instead of implementing?
Always assess first when the clinical picture is incomplete or ambiguous. If a patient reports a change in status, you must validate that change with an assessment before taking an invasive action.
How does BlueMint RN help if I am still getting prioritization questions wrong?
